DC2R O2 sensor wiring

Hi guys,

I need some urgent help here. I have noticed that my O2 sensor wiring has all come out of the O2 Sensor some how. I have been running a PowerFC so the O2 sensor wasn't turned on. Now that I am reverting back to the Stock ECU, I need to find out what wire goes into what hole in the O2 sensor. There are 4 wires; from my crap memory, it's 2 Black, 1 Green and 1 White.
Can someone tell me 100% where these go, and if possible if you have a pitcure it would be alot better.
